Welcome to Commonwealth Primary Care / Family Medicine
Our primary care office provides excellent health care to patients of all ages, including babies, children, adolescents, adults, and older adults. We work with our patients to help manage chronic diseases, to care for urgent problems, and to maintain good health through preventive care.
